Me First - Transitioning to Adult Care

In healthcare, 'transition' describes the process of preparing, planning and moving from children’s to adult services. The University Hospital Southampton NHS Foundation Trust has developed the Ready Steady Go programme to support young people through this difficult time and to encourage them to get involved in the transition process to make them feel confident and happier about the change.

This is mainly a training resource for HCPs to use with transition with good resources to use in own units.

This website provides some very useful information about what to expect from the transition period. It's very practical in terms of the user being able to download plans which will assist them in planning for their appointments.
